Sleep disorder, investigated and managed
Hospital course note
Dear colleague, I am writing to inform you regarding the discharge of your 56-year-old
male patient admitted on 13.12.2012 for evaluation of sleep disorder. During his brief
inpatient stay, we conducted a comprehensive sleep assessment including detailed sleep
history, Epworth Sleepiness Scale scoring, and overnight polysomnography to objectively
characterize his sleep disturbance. Investigations revealed findings consistent with
unspecified sleep disorder (ICD-10: G47.9), without evidence of significant obstructive
sleep apnoea requiring immediate intervention. We initiated sleep hygiene counselling
and provided structured relaxation guidance. A short-term low-dose pharmacological
support was prescribed to stabilize sleep pattern. We recommend outpatient follow-up
with sleep specialist review of polysomnography results. Please find enclosed the
