Dear colleague, I am writing to inform you regarding our mutual patient, a 66-year-old
female with a known history of coronary artery bypass grafting, who was admitted for
routine post-CABG follow-up assessment. During her hospital stay, we performed a comprehensive
cardiac evaluation including electrocardiography, echocardiography, and routine laboratory
investigations. Chest X-ray was obtained to assess cardiopulmonary status. Her cardiac
function was reviewed and found to be stable, with no signs of graft failure, arrhythmia,
or decompensated heart failure. Current medications were reviewed and optimised accordingly.
She was discharged in good general condition with continued antiplatelet and statin
therapy. Please arrange follow-up in approximately six weeks and monitor lipid profile
