Pre-renal transplant workup completed, patient listed
Hospital course note
Dear colleague, we are writing to inform you regarding your patient, a 56-year-old
male admitted for pre-renal transplant evaluation. During his stay from 29th to 31st
December 2008, a comprehensive transplant workup was completed. This included renal
function assessment, cardiovascular evaluation, serological and infectious disease
screening, immunological typing and cross-matching, as well as urological and general
surgical fitness assessment. Imaging studies including renal ultrasound and chest
X-ray were performed. The patient demonstrated acceptable surgical and medical fitness
for transplantation. All required preparatory steps have been fulfilled and the patient
has been formally listed on the kidney transplant waiting list. Dialysis preparatory
care planning has been initiated as per protocol (ICD-10: Z49.0). We recommend continued
close monitoring of renal function and cardiovascular status in your practice pending
transplant availability. Please do not hesitate to contact us with any queries.
