Substance dependence, treated and stabilised
Hospital course note
Dear colleague, I am writing to inform you regarding the discharge of your 75-year-old
male patient, admitted on 11/01/2023 due to substance dependence. Upon admission,
a comprehensive psychiatric and somatic assessment was performed, including structured
addiction history, laboratory screening, and physical examination, confirming multiple
substance dependence syndrome (ICD-10: F19.2). During the brief inpatient stay, the
patient underwent medically supervised detoxification with pharmacological support
to manage withdrawal symptoms safely. Vital signs were closely monitored throughout.
Psychoeducational counseling was initiated, and the patient was motivated toward ongoing
outpatient addiction treatment. At discharge on 13/01/2023, the patient was medically
stable with no acute withdrawal complications. I strongly recommend referral to an
outpatient addiction specialist and close follow-up of his general medical condition.
