Dear colleague, I am writing to inform you regarding the discharge of your patient,
a 73-year-old male, admitted for routine post-coronary artery bypass grafting follow-up
assessment. During his brief inpatient stay, we performed a comprehensive cardiac
evaluation including electrocardiography, echocardiography, and routine laboratory
investigations. His cardiac status was reviewed and found to be stable, with no evidence
of graft failure, ischaemic changes, or significant arrhythmia. Blood pressure and
heart rate were within acceptable ranges throughout admission. Current cardiac medications
were reviewed and continued unchanged. The patient was ambulatory, haemodynamically
stable, and without complaints of chest pain or dyspnoea at discharge. We recommend
continued outpatient follow-up with cardiology, adherence to current medication regimen,
and lifestyle modifications including cardiac rehabilitation if not already initiated.
