Dear Colleague, I am writing to inform you regarding your patient, a 56-year-old male
admitted on 07/04/2023 for routine post-coronary artery bypass grafting follow-up.
On admission, a thorough cardiac assessment was performed, including electrocardiography,
echocardiography, and laboratory investigations. Cardiac biomarkers were within normal
limits, and haemodynamic parameters remained stable throughout his stay. Graft patency
was considered satisfactory based on clinical and non-invasive findings. No signs
of post-operative complications, including wound infection, arrhythmia, or heart failure,
were identified. Medication reconciliation was performed, and current cardioprotective
therapy was reviewed and optimised. The patient was counselled regarding lifestyle
modifications, cardiac rehabilitation attendance, and warning signs requiring urgent
review. He was discharged on 08/04/2023 in stable condition. Please continue routine
