Dear colleague, we are writing to inform you about the inpatient stay of your 69-year-old male patient, admitted for pre-transplant kidney evaluation. During his stay, we completed a comprehensive pre-renal transplant workup including renal function assessment, cardiac evaluation, serological and immunological screening, imaging studies, and dialysis access planning. Cross-match and HLA typing were performed and submitted to the transplant registry. Infectious disease clearance was obtained. Nutritional and anesthesiological risk assessment were completed without contraindications to transplantation. The patient has been formally listed for deceased-donor kidney transplantation. Dialysis preparatory care planning (Z49.0) has been initiated accordingly. We recommend continued close monitoring of renal function and blood pressure in your practice.
