Dear colleague, I am writing to inform you regarding your patient, a 54-year-old female
who was admitted on 02.04.2002 following her voluntary request for surgical sterilization.
Prior to the procedure, a thorough preoperative assessment was conducted, including
a review of her medical history, physical examination, routine blood work, and anaesthetic
evaluation. The patient confirmed her informed consent for permanent contraception.
She underwent laparoscopic bilateral tubal ligation under general anaesthesia on 03.04.2002,
which was performed without complications. Postoperative recovery was uneventful,
and the patient remained haemodynamically stable throughout her stay. She was discharged
home on 04.04.2002 in good general condition with standard postoperative instructions.
No further gynaecological follow-up is anticipated unless symptoms arise. Please do
