Substance dependence, treated and stabilised

Hospital course note

Dear colleague, I am writing to inform you regarding the discharge of your 24-year-old female patient, admitted on 10.05.2025 due to substance dependence disorder. Upon admission, a comprehensive psychiatric and somatic assessment was conducted, confirming dependence on multiple substances including opioids and benzodiazepines. Urine toxicology screening was positive for multiple substances. The patient underwent a structured detoxification protocol with medically supervised withdrawal management, including symptomatic pharmacotherapy to address withdrawal symptoms. Psychological counseling sessions were initiated during the inpatient stay. The patient was stabilized successfully without major complications. At discharge, she was motivated for continued outpatient addiction treatment. We strongly recommend referral to an outpatient addiction counseling service and close follow-up including regular urine drug screening. Should any concerns arise, please do not hesitate to contact our department.
