Dear colleague, I am writing to inform you regarding the discharge of your 34-year-old male patient, admitted on 08/08/2020 due to substance dependence. Upon admission, a comprehensive psychiatric and somatic assessment was conducted, including laboratory screening, urine toxicology, and structured addiction severity evaluation. The patient met criteria for ICD-10 F19.2, dependence syndrome related to multiple drug use. During the inpatient stay, a medically supervised detoxification was initiated, alongside pharmacological support to manage withdrawal symptoms. Psychological counselling sessions were provided, and the patient was introduced to a structured motivational intervention programme. Social work assessment was completed to address psychosocial risk factors. The patient was discharged in a stabilised condition on 19/08/2020.
