Dear colleague, I am writing to inform you regarding your patient, a 62-year-old female
who was admitted on 25 September 1996 at her own request for voluntary surgical sterilization.
Prior to the procedure, a thorough preoperative assessment was performed, including
clinical examination, routine laboratory workup, and anaesthetic evaluation, all of
which returned satisfactory results. The patient confirmed her informed consent following
detailed counselling regarding the permanent nature of the procedure. On 25 September
1996, bilateral tubal ligation was performed laparoscopically under general anaesthesia
without intraoperative or postoperative complications. Recovery was uneventful and
the patient was discharged home on 26 September 1996 in good general condition. No
further gynaecological follow-up is required unless the patient presents with concerns.
